Imaging of the Geriatric Brain

Structural neuroimaging almost invariably shows significant abnormalities in most neurodegenerative disorders. The most prevalent neurodegenerative disorder-Alzheimer's disease (AD)- is one of the leading health concerns of the twenty-first century, with an increasing elderly population and its exponentially increasing social and economic impact. This chapter explains the role of structural neuroimaging in AD and in the frontotemporal dementia (FTD) spectrum. The role of structural neuroimaging in dementia with Lewy bodies and in Parkinson's disease dementia is also explained. New advancements in functional imaging may provide tools for identifying neurodegenerative brain disease for clinical decision making and treatment development. The chapter surveys the current amyloid imaging techniques using 11C-labeled (11C) agents. In the future, amyloid imaging is likely to supplement clinical evaluation in selecting patients for anti-amyloid therapies, while magnetic resonance imaging (MRI) and fluorodeoxy glucose-positron emission tomography (FDG-PET) may be more appropriate markers of clinical progression than cognitive tests.
